What are the best places on the Nicoya Peninsula to retire? What might a monthly budget for fairly simple living be?

If access to health care is important to you, Nicoya is not a great choice. I know the idea of retiring close to the ocean is appealing, but Nicoya is basically a wild west environment.
If services are at all important to you, you should consider something closer to the Central Vally. Beach communities are sorely lacking in the types of services that the typical retiree are looking for.
